My goal in completing a bariatric evaluation is to ensure that you are aware of the potential risks and benefits of your chosen procedure, that any mental health symptoms are effectively managed, and that you are in a place in life where you are able to make significant behavioral changes to your nutrition and fitness. A pre-surgical bariatric evaluation should help set you up for success as you prepare for and recover from surgery.
Your bariatric evaluation includes:
Assessment forms and screening tools
Two sessions for evaluation and feedback, with homework assignments completed in between
Report or letter sent to your bariatric surgeon and/or insurance company
